DON'T FORGET! When you register for camp, download the Health Form and Welcome packet.
This health form must be completed and returned before your child attends camp. Send to: SCC, 4601 Mid Rivers Mall Drive, Cottleville, MO 63376 Attn: HUM/103.
IF YOUR CHILD HAS AN EPI-PEN OR INHALER PLEASE MAKE SURE TO BRING IT IN A LABELED BAG. IF HE/SHE SELF ADMINSTERS WE MUST HAVE A DOCTORS SIGNATURE ON THE HEALTH FORM. EVEN IF YOU DON'T THINK YOUR CHILD WILL NEED IT, WE NEED TO HAVE IT AT CAMP FOR YOUR CHILD TO PARTICIPATE. YOUR CHILD'S SAFETY IS OUR #1 PRIORITY!
A $25 cancellation fee will be assessed if you cancel less than 14 days prior to the start date of camp.
